Alliant Home Health Acquired by Choice Health at Home — Healthcare M&A

Choice Health at Home has completed the acquisition of Alliant Home Health, a home health business in Arizona. Choice Health at Home acquisitions bring Alliant’s skilled nursing, physical therapy, and occupational therapy services for post-acute and chronic patients into its home-based care platform. The Alliant Home Health acquisition also adds palliative care and hospice programs, supporting integrated care for patients and families across Arizona. The add on acquisition closed on November 12, 2025, with undisclosed financial terms, and expands Choice’s geographic footprint in the state. Backed by private equity, Choice Health at Home pursued the home health acquisition as a healthcare M&A growth step, while Alliant sought scale and sustainability through the private equity acquisition.
